States Semen Shipment) or international (International
Semen Shipment) locations. Our services also include rental
of shippers (Rent a Vapor Shipper), request
for evaluation of collection (Canine Semen Evaluation) and shipment of samples for DNA
evaluation (DNA Testing Shipment Request).

The shipment forms may be downloaded and printed for use They must be
complete including signatures or shipment will not move
forward.
We require two business days for domestic shipments
and four weeks for most international shipments. Planning ahead
will ensure availability of shippers, allow for proper paperwork preparation
time and eliminate urgent rush fees.

The Domestic Shipment Request Form is to be used for shipments within the
United States and provides all the information needed regarding shipping
requirements, fees and forms of payment.

Description of the service
The International Shipment Request Form provides all the information needed regarding
shipping requirements, fees and forms of payment. It is the responsibility
of the importer to check with the appropriate government agencies to obtain
the proper requirements necessary for the importation of canine frozen semen
into his or her country. Canadian shipments require no additional paperwork
to fulfill shipping requirements.

notice upon receipt of the completed form is required in order to avoid the assessment
of any rush fees. Please allow two business day to ship to Canada to avoid rush
fees.

Description of the service
Synbiotics Freezing Documents are required for authorization to transfer
and store semen with Synbiotics and also provide necessary information
for future storage billing, shipments and breedings.

appointment.
The Synbiotics Storage Contract should be fully reviewed before signing.The Telephone Release Form allows Synbiotics to release your Stud Dog’s Collection information when the security information is correctly provided and helps us address customer inquiries.Record of Collection Ownership documents ownership of the semen collection at the time of collection. Stud dog owner(s) signature(s) need to be provided to a Synbiotics Freeze Center Veterinarian before collection occurs. This is critical for registration of AKC litters.
